Adult Day Care

Our Adult Day Care program provides structured daytime supervision in a safe, supportive environment.

Services include:

  • Supervised daytime care
  • Health observation and wellness monitoring
  • Assistance with activities of daily living (as authorized)
  • Medication reminders (as permitted)
  • Social, recreational, and cultural activities
  • Nutritious meals and snacks
  • Ongoing communication with families and care teams
  • Senior Housing Counseling. Help with finding housing.
  • Help with applying for FoodShare and Medicaid Education
  • Senior Community Garden.

NUTRITION & SOCIAL CONNECTION

Participants receiving Adult Day Care services enjoy nutritious meals in a communal setting that promotes routine, socialization, and well-being.

Shared meals help reduce isolation and strengthen community connection.

WHO WE SERVE & ELIGIBILITY

Eligibility & Referrals

Participants may be eligible if they:

  • Are age 60+ or meet program criteria
  • Benefit from in-home support, daytime supervision, or transportation
  • Can safely participate in services
  • Are enrolled in or eligible for Family Care, Partnership, IRIS, Private Pay, or PACE

     

  • Are referred through ADRC, MCO, hospital, or care team

All services are authorization-based and coordinated with care managers.
